      Use this assignment to submit your sick leave certificate or relevant medical documents if you were unable to attend a class. Once submitted, the instructor will review your request and update your attendance record accordingly. Important Notes: Only submit official documents (PDF, image, or scanned copy). Ensure the document clearly shows the date(s) you were absent. Submissions are confidential and only accessible to the course instructor/admin. After submission, you will receive a confirmation, and once assessed, you will be notified if your absence is marked as “Excused” or “Not Excused.”
